This spring his former pain management doctor dropped him suddenly leading to what he said was a horrible withdrawal. Chronic pain patients such as Greene are becoming increasingly vocal about what they say is a medical community that’s shutting them out.
The Centers for Disease Control and Prevention issued guidelines in 2016 to cut back prescriptions after years of liberal opioid dispensing contributed to addiction and overdose deaths. Those guidelines not only influenced doctors – state regulators, health insurers and even disability administrators also have cited these federal guidelines to justify policies that limit pain pill prescriptions. Last month, the CDC clarified its position, saying that the response to the opioid crisis went too far.

FALL AND BALANCE PROGRAM
Our Falls and Balance Program runs over a 6 week cycle, with patients able to commence the program at any time.
To begin the program, participants attend a 60 minute initial assessment with our Physiotherapist to determine the contributing factors for the participants increased falls risk. Following their initial assessment the patients can commence the Falls Prevention exercise and education program.
At the end of their 6 week program the patient will have a 30min session with the physiotherapist for reassessment, to determine improvements made and any ongoing areas of weakness or concern. Classes can be continued after the initial 6 weeks if patient or Physiotherapist deems appropriate.
Although we can't see them, our bones need TLC! Having adequate amounts of protein, calcium and vitamin D depending on age and gender can reduce the risk of osteoporosis and prevent further bone loss.

Retinal Pigment Epithelium (RPE) Tear.

RPE tear associated with vascularised RPE detachment.
📖 RPE tears were first reported in 1981 by Hoskin et al. as a complication in patients with PED due to AMD. RPE tears can be either part of the natural course of PED due to occult CNV, retinal angiomatous proliferation, or polypoidal choroidal vasculopathy or they can occur in association with various treatments for neovascular AMD such as photodynamic therapy, laser photocoagulation, transpupillary thermotherapy and anti-VEGF’s therapies.
Using funduscopy, an RPE tear may be suspected based on the presence of subretinal and sub-RPE blood, an often well-defined hyperpigmented line at the site of the rolled RPE, and a depigmented area corresponding to the exposed choroid. Retinal or subretinal haemorrhages are clearly distinguishable from RPE tear areas in colour fundus photography (CFP) compared to other modalities. Haemorrhages may interfere with an RPE tear area detection in fundus autofluorescence (FAF), which makes CFP indispensable in this entity.
In FAF imaging, areas of RPE tears exhibit a markedly reduced autofluorescent signal because RPE cells with fluorescent lipofuscin are lost. Small RPE tears are more evident in FAF and are easier to detect than in CFP. The high contrast of these hypoautofluorescent areas, compared with the intact retina, allows for an easy and accurate determination of lesion boundaries.
In a classic RPE tear, three parts of the site can be distinguished on SD-OCT scans. In the first part, the outer retinal bands show a normal architecture. In the second part, the retina forms a dome-shaped detachment from the choroid. In this elevated zone, SD-OCT shows the free edge of a wavy, contracted RPE being retracted towards the CNV. The rolled RPE causes a hyperreflectivity and an intense back-shadowing, which completely masks the choroid. In the third part, the RPE is completely absent, and the neurosensory retina appears thinned.

DAD UPDATE: Unfortunately, Dad is back in the hospital. His heart function is good, but he had a rough night at home and we had to take him to a nearby ER this morning, then he was taken by ambulance back to the hospital where he had surgery. He has a great heart surgeon team, and they are leading his care again now so hopefully he’ll be back on track soon. Mom and I are holding up OK on minimal sleep in the midst of everything, and I’ve learned multiple times in the past 24 hours that 170ish human pounds seems heavier than the same amount in metal weight plates. And somehow, I’ve taken one online grad school test today in the ER, and I’ll take one more by midnight. Thanks for your continued thoughts and prayers. You know we Never Give Up. 🙏🏽❤️💯
I really like this pyramid. Not because it’s 100% accurate, but because it’s reasonable and backed by science. We could get into discussions about where sleep, weight lifting, cardio, and the other factors should fall on this pyramid but ultimately the information presented is non-extremist, reasonable, and most important backed by a solid evidence base.
Now some people might say that I shouldn’t be posting any nutrition related information because I’m a physical therapist and that’s out of my scope of practice.
However I would argue that understanding how to science and having a decent comprehension of how the body utilizes and stores energy is not relegated simply to dietitians.
I would never try to develop a specific nutrition plan to treat someone’s nutrition related disorder. This is outside of my scope and not how I can personally best serve someone.
However, a foundational Understanding of nutrition and when/why to refer someone does become exponentially more relevant when we read and recognize that much of the literature is showing us that metabolic syndromes and excessive body weight are key contributors to musculoskeletal injuries and many people’s experience of pain.
